St Vincent's Health & Aged Care (SVHAC) is a Catholic not for profit private health and aged care organization. As a group of Catholic private hospitals and aged care facilities, SVHAC reflects in its policies and practices the ethical and moral teachings of the Catholic Church. Other information you need to be aware of before completing your application;
There are three levels of accreditation at our Facilities. Please follow the links to complete the appropriate Application:
Level 1 - awarded to all Accredited Practitioners in accordance with By-Law 10. This includes a selection of Authority to Release Information forms. Depending on which insurance provider you use, please complete and sign the appropriate form and return with your application. This will enable us to gather relevant insurance information, which is required as per Private Hospital By-Laws, directly from your insurance company in the future, which saves you from having to submit this information on an annual basis. Level 2 - for when a VMO who is already an Accredited Practitioner wishes to undertake a particularly specialised procedure or service delivery for his/her first time at the Facility and within his/her defined specialty. Click here for Application Form
Level 3 - for when a VMO who is already an Accredited Practitioner wishes to undertake a new interventional or surgical procedure or clinical service which will occur for the first time at the Facility in accordance with By-Law 31. Click here for Application Form
Completed Applications should be mailed to PO Box 555, Spring Hill QLD 4004, or fax to 07 3852 6943. You will note under these By-Laws applications will not be processed unless they include the following: -
- a copy of your current Queensland Medical Registration Certificate
- a current insurance policy schedule or insurance certificate (on company stationery) stating all of the following information
- the specialty covered
- Policy Claim Limit; for example $20 Million
- the period of insurance; for example 1 January 2011 to 31 December 2011
- a copy of your Curriculum Vitae
- three formal written references
Please also ensure that your chosen three (3) Professional Referees are credentialed with at least one (1) SVHAC facility, and preferably represent the same specialty as your own.
